The detecting of sensing values and pacing threshold is a fundamental operation at the time of implanting or replacing a permanent pace-maker in human heart pathologies.
For this purpose the analysis protocol foresees the use of an instrument that, for these reasons, is known as a threshold analyser. During the heart activity procedure it is essential for the patient to be monitored continuously by additional recording instruments (electrocardiograph or other). Finally, a defibrillator must be on hand for dealing with any deterioration of the patient’s conditions.
The 2307 is a portable, single-chamber, battery-run threshold analyser, designed to allow for simple yet precise measuring of the sensing values and pacing threshold.
In fact, it allows for measuring the following in each preselected heart chamber:
· Peak-peak value of the amplitude of the electrogram detected in mV (minimum and maximum)
· Slew rate of the electrogram detected in mV/ms (minimum and maximum)
· Stimulation threshold in V
· Impedance value at 5V of the patient circuit in Ω.
The 2307 is fundamentally a programmable medical device serving for diagnostic purposes and not for life support and which, for these measures, envisages a series of functions that can be selected from a main menu and are described in detail in the relative paragraphs of chapter 7. Moreover, for safety reasons in the conduction of the clinical procedure, the device has been fitted with a specific emergency stimulation function that can be activated from a specific control.
The diagnostic functions envisaged in the device for detecting the useful parameters for programming a permanent heart stimulation are four and these can be identified from the following indications:
Auto – Sense (function for measuring the sensing parameters with automatic detection by the electrogram)
Pace threshold S00 (function for measuring the threshold via asynchronous stimulation)
Pace threshold SSI (function for measuring the threshold via stimulation on request)
5V Impedance (function for measuring the impedance of the stimulation circuit via asynchronous stimulation)
